- Analysis of Characteristics Microvessel Density of Thyroid Malignant and Benign Nodules on Contrast-Enhanced Ultrasonography

Objective: To analyze the contrast-enhanced ultrasonography (CEUS) characteristics and microvessel density (MVD) of thyroid carcinomas vs. benign thyroid lesions, and to determine the value of CEUS in the clinical diagnosis of thyroid carcinoma.
Methods: Forty-five patients (34 women, 11 men; mean age, 45.95 ± 12.20 years) with 57 thyroid nodules underwent preoperative CEUS with SonoVue (injected as a bolus via an elbow vein). The CEUS data were analyzed using QLAB quantitative analysis software. The enhancement patterns and quantitative parameters of thyroid carcinoma were evaluated.Additionally, immunohistochemistry was performed to evaluate MVD.
Results: Most malignant nodules showed hypo-enhancement, and this enhancement pattern had 68.18% sensitivity and 92.31% specificity in diagnosing thyroid carcinomas. Most benign nodules showed ring enhancement, which had 46.15% sensitivity and 100% specificity in diagnosing benign nodules. Compared with the peripheral glandular tissue, thyroid carcinomas had significantly smaller peak intensities (PI) and wash-in slopes (P < 0.05). Malignant nodules had significantly lower MVD than benign nodules (P < 0.05). A positive correlation was between the PI and MVD (P < 0.01).
Conclusion: CEUS can improve the diagnosis of thyroid carcinomas, and is a useful complement to conventional ultrasonography.

Objective: To evaluate the correlation between traditional ultrasound and the expression of estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor-2 (Her-2), and Ki-67 expression in breast carcinoma.
Methods: A retrospective analysis of 400 cases of breast cancer, which were confirmed by pathology. The expression of postoperative specimens by immune-histochemical staining was used to evaluate the immune molecules ER, PR, Her-2, and Ki-67. The correlation between the ultrasound findings (size, boundary, sand calcification, and blood flow grade), as well as the expression of ER, PR, Her-2, and Ki-67 were analyzed.
Results: Of the 400 cases, there were no correlations between the boundary, the classification, or the blood flow of the tumor with the expression of ER, PR, Her-2, and Ki-67. The size of tumor was correlated with Her-2 and Ki-67. The tumor is the bigger, and the expression of Her-2 and Ki-67 is higher.
Conclusion: There is a correlation between tumor size and expression of Her-2 and Ki-67 in breast cancer. The combination of them was important for the treatment and prognosis of breast cancer.

- Acute Transient Thyroid Swelling after Fine-needle Aspiration Biopsy: Case Report and Review of the Literature

The complications of thyroid fine needle aspiration biopsy (FNAB) have been long observed, but the knowledge of some rare complications is still improved. In this case report, a rare complication of acute transient thyroid swelling (ATS) shortly after FNAB was reported for the first time in the Chinese mainland. The patient showed an obvious swelling of the neck and suffered from acute pain less than 20 minutes after aspiration. Immediate thyroid ultrasound examination showed diffuse swelling of the entire thyroid gland with heterogeneous echo changes. 17 hours later, the repeated examination showed the thyroid was restored. Pathology of FNAB showed some atypical thyroid follicular epithelium and the final pathology showed it was papillary carcinoma. We also reviewed the literature and preliminary concluded that ATS was a sudden and rare complication of FNAB with a rapid self-repair result, while the reason for ATS was still unknown and further puncture for the diffuse altered area is necessary to explore the physiopathologic mechanism.

Epithelioid angiosarcoma is a very rare low-grade vascular neoplasm. We report a case of a 61-year-old woman who suffered it in the left atrium. This case is instructive in challenging our understanding of cardiac space-occupying lesions and highlighting the importance of careful clinical evaluation.

Primary hyperparathyroidism (PHPT) is a disorder of one or more of the parathyroid glands, which results in hypercalcemia. Hyperparathyroid crisis is a rare condition that sometimes occurs when people with PHPT experience another illness, like vomiting or diarrhea, which causes excessive fluid loss or severely limits the amount of fluid they can consume. Parathyroid adenoma and hyperplasia are the most common causes of PHPT. In clinical practice, functional parathyroid cysts are a very rare cause of PHPT. We report a rare case of a functional parathyroid cyst as a cause of hyperparathyroidism or even hyperparathyroid crisis. After surgery, the patient's blood calcium and PTH level returned to normal ranges. Histological examination showed in fibrous cystic wall, some of the inner wall of the capsule were covered with a single layer of flat epithelium, some were not covered with cells, and a small cluster of parathyroid tissue was extruded and deformed in the wall of the capsule.

Behçet's disease is an uncommon multisystem inflammatory disease characterized by oral and genital ulcers, skin lesions, and inflammation of the eyes. We reported the case of a young man with Behçet's syndrome who presented with severe aortic regurgitation that mimicked infectious endocarditis and required reoperation.
